What is the NDIS?
Support Coordination is a capacity-building support designed to help participants understand and implement their NDIS plan. It focuses on connecting participants with appropriate providers, coordinating supports, and building the skills needed to manage services independently over time.
A Support Coordinator works alongside participants, families, and providers to ensure supports are aligned, effective, and working toward the participant’s goals. This includes assisting with service agreements, resolving challenges, and adapting supports as needs change.
Support Coordination can be particularly valuable for participants who have multiple supports in place, are new to the NDIS, or require assistance navigating complex systems.

3. Participants with multiple supports who need assistance coordinating services and ensuring everything is working together.
4. Participants who want to build independence and confidence in managing their supports over time.
Support Coordination can be tailored to a wide range of needs, from simple guidance through to more complex coordination.
